Follow these steps to create your username and password. Then log in to view your benefits complete your enrollment.

1. Select “Create an Account”.
2. Enter your last name, birthdate and the last 4 digits of your social security number to verify your identity. Click Next.
3. Set your username and password and create your security questions. Click Next.

Please note that you must create your username and password from a web browser.  Once your account is created, you may enroll on the mobile app.
